Nurse and Wellness

Here at the Murphy Elementary School District #21 has either a licensed RN or trained health technician that delivers nursing services for all students enrolled. There is a health office at each school in the district. The nursing staff delivers (as needed) first aid, administers daily physician-prescribed medication with proper approval, provides comfort care, and offers referrals to community-based health services which include immunizations, eyeglasses, and physical check-ups. The nursing staff also meets all health-related needs of students with special needs with an IEP, Section 504 Plan, or Health Service Plan and related services in special education. Several of our campus health offices are equipped to offer free immunizations to any students in the district. Some of our sites sponsor free health clinics run by outside providers. A lead nurse (RN), located at Kuban Elementary School, monitors all health services in the school district.

Immunizations

The laws and rules governing school immunization requirements are Arizona Revised Statutes §15-871-874; and Arizona Administrative Code, R9-6-701–708. Students must have proof of all required immunizations in order to attend school. Parental recall or verbal history of any disease is not accepted; therefore students who are not vaccinated must submit an ADHS medical exemption form. Vaccines must follow minimum intervals and ages to be valid. Arizona law allows K-12 immunization exemptions for medical reasons, lab evidence of immunity, and personal beliefs.

Arizona State law now requires all students, 11 years of age to have their:

  • Quadrivalent Meningococcal vaccine (MCV4)
  • Tetanus/Diphtheria vaccine (Tdap)

Student Wellness

Communicable Disease:

A student suffering from a communicable disease shall be excluded from school to protect the student's own welfare and also to protect other students from illness. Any student with, or recovering from, a communicable disease will not be permitted in school until the period of contagion is passed or until a physician recommends a return, in accordance with A.R.S. 36-621 et seq.

Pediculosis (Head Lice):

Students with pediculosis shall be excluded from school until treated and there is no live lice. Students will be checked by nurse staff before returning to class. Student are checked for head lice periodically throughout school year.

Medication

Prescription Medication:

All prescription medication for students MUST be provided by the parent in the original package, a written order from the physician stating the name of the medicine, the dosage, and the time it is to be given, and a SIGNED district form. School nurses can provide limited OTC (Over the Counter) medications with consent on the student's Emergency Card. All emergency medication will be stored in the nurses office but follow the student when traveling on field trips, staff will be trained annually on medication procedures.

 

Allergies:

Please fill this form out if your child has an allergy and takes Benadryl or has an Epi-Pen. You can either print them out or fill them out in the nurse's office at your child's school.

 

Asthma:

If your child has Asthma please fill out the Asthma Action Plan seen below and have your child's doctor sign it. Please return this form to the nurse's office at your school. These forms are good for one school year.

Health Screening

Students are screened throughout the year according to state guidelines. Students are screened in vision and hearing. Referrals will be sent home to parents for students who may need further evaluation by a physician. If you have concerns or health information regarding your student, please contact the nurse at their school.
